Health Project Associate Responsibilities:
- Assist with project implementation based on defined plans

Clinic Hub and Health Outreach Coordinator

The Position
We are hiring a driven and detail-oriented individual to serve as our Hub and Outreach Coordinator.

The Position
As a Health Coach, you’ll offer direct virtual and physical support to members managing chronic health issues like diabetes and hypertension using our unique model.
Your duties will include both fieldwork and online support.
You must follow evidence-based practices while adapting them to member needs.
You will work with a team to help members achieve their wellness goals, responding mostly online, and occasionally through live chats.
You will support individuals to take charge of their health using guidance and tools to make lifestyle changes.
